| The Guelph Community Health Centre is located downtown Guelph at the intersections of Woolwich, Eramosa and Wyndham Streets. Parking is in the Baker Street Public Parking located behind the Health Centre on Baker Street. Access is also available through Guelph Public Transit System, St. George's Square (Main transfer stop for all busses).
